COVID-19 VACCINE CLINIC
The Lake Hamilton School District will host a free, voluntary, COVID-19 vaccination clinic in the lobby of Wolf Arena on Wednesday, April 28 from 9:00 a.m. until 12:00 p.m. The vaccination clinic has been coordinated through the Garland County COVID-19 Task Force.
๐๐๐ฎ๐ถ๐น๐ฎ๐ฏ๐ถ๐น๐ถ๐๐: The vaccine clinic is available to all Lake Hamilton students 16 years and older, as well as Lake Hamilton family members. A parent/guardian must be present for all student vaccinations.

ENVIRONMENTAL KINDNESS DAY

REMINDER | WELL CHILD VISIT
All children who receive Arkansas Medicaid services must see their doctor for a well child visit each year.
Families needing to establish a primary care physician can visit (
https://afmc.org/individuals/arkansans-on-medicaid/connectcare/ ) or call the Lake Hamilton School District Special Services office (501-767-8444) for assistance.
For those wanting to visit the Wolf Health Clinic for their well child visit that are not current Healthstar patients must receive a referral from their doctor for that service.

REMINDER | STUDENT DIGITAL DAY
Friday, April 16 is a student digital day. Students will not attend school onsite this day. Instead, they will complete virtual assignments from home. Attendance will be counted based on the completion of assignments. Teachers will be onsite during Student Digital Day and will communicate specific participation expectations with students.

VACCINE INTEREST SURVEY
Arkansas Governor Asa Hutchinson announced Tuesday that all Arkansans age 16 and above are now eligible for the COVID-19 vaccine. The Lake Hamilton School District is looking to host a free, voluntary, on-site vaccination clinic for all parents of LHSD students and students age 16 and older. The District will use this survey (
https://lhsd.info/Vaccine-Interest-Survey) to gauge how many of our parents and students (16 and older) would like to be vaccinated if an on-site option was scheduled.
Please fill out the survey if you and/or your student (16 or older) would be interested in getting the COVID-19 vaccine at an on-site clinic held at Lake Hamilton.
Please note: Any student that is 16 or 17 years of age must have a parent/guardian present in order to receive the vaccine. Students who are 18 years or older may receive the vaccine without a parent/guardian present.
This is not a registration form for the vaccine. This is an interest survey. The deadline to complete the interest survey is Monday, April 5, 2021. Depending on the response, the District will work to schedule an on-site clinic for those that are interested in the very near future.
