The Lake Hamilton School District will host a free, voluntary, COVID-19 vaccination clinic in the lobby of Wolf Arena on Wednesday, April 28 from 9:00 a.m. until 12:00 p.m. The vaccination clinic has been coordinated through the Garland County COVID-19 Task Force.
๐๐๐ฎ๐ถ๐น๐ฎ๐ฏ๐ถ๐น๐ถ๐๐: The vaccine clinic is available to all Lake Hamilton students 16 years and older, as well as Lake Hamilton family members. A parent/guardian must be present for all student vaccinations.
๐ช๐ต๐ฎ๐ ๐ง๐ผ ๐๐ฟ๐ถ๐ป๐ด: Please fill out the consent form (https://lhsd.info/Vaccine-Consent-Form), bring a photo ID, and an insurance card (if applicable). There is ๐ป๐ผ ๐ฐ๐ต๐ฎ๐ฟ๐ด๐ฒ for the vaccine.
